Love of My Life Terms and Conditions

Liability I, (The Client), hereby release Carey Weiss from any liability or claims that could be made against her concerning my mental and/or physical well-being during the work that has been outlined and agreed upon (now and in the future). This liability waiver is not intended to exclude or restrict liability for death or personal injury caused by negligence.

Scope of Practice

I understand that Carey Weiss is not a licensed physician, psychologist, and is not functioning as a medical practitioner of any kind and that hypnotherapy or life coaching should not be considered a replacement for the advice and/or services of a psychiatrist, psychologist, psychotherapist, or doctor.

Participation

I give Carey Weiss full permission to hypnotize me and to use Rapid Transformational Therapy® knowing that by participating fully in the process and by listening to my personalized recording for 21 days, I play an important role in my overall success. I recognize that my success in this program/partnership is based on my efforts and willingness to be an active and eager participant in my own growth and development.

Guarantee

I understand that although Rapid Transformational Therapy® and Transformational Life Coaching has an incredibly high success rate, Carey Weiss cannot and does not guarantee results since my own personal success depends on many factors that Carey Weiss has no control over, including my willingness and desire to effect the changes inside myself.

Audio Recordings

I give Carey Weiss full permission to make audio recordings that may include my voice. I understand that if a recording (or recordings) is made during or after my session(s) Carey Weiss retains full copyright over any forms of media that may be produced and distributed to me.

Deepening Process

If applicable; I hereby grant permission to Carey Weiss to respectfully lift my arm, touch my shoulder, or rock my head (when applicable for in-person sessions) during my Rapid Transformational Therapy® session(s) in order to help facilitate the deepening process.

Confidentiality

Carey Weiss may release information to a specific individual or agency if it has been determined that a vulnerable person (child or adult) is at risk; if I, as a client, am in imminent danger to myself or others; or if a subpoena of records has been requested. I also understand that, at any time, Carey Weiss may discuss aspects of my case with other colleagues, keeping my full name and identity completely confidential always unless I have given permission otherwise. 

Process

Any mention of drugs in the course of consultation is only for the purpose of providing a complete history of drugs that the client is taking and not for Carey Weiss to judge the appropriateness of the medication. Any change in prescription or dosage is a decision the client makes with his or her physician. Rather than dealing with treatment of disease, Carey Weiss focuses on wellness, healing and prevention of illness through the use of non-toxic, natural holistic therapies to achieve optimal health. As a Certified Hypnotherapist and Life Coach, Carey Weiss primarily educates and motivates clients to assume more personal responsibility for their health by adopting a healthy attitude, lifestyle, and diet. I acknowledge that while people generally experience greater health and wellness as a result of embracing a healthier attitude, lifestyle, and diet, Carey Weiss does not promise or guarantee protection from future illness. 

I understand that Carey Weiss is a Hypnotherapist and Life Transformation Guide and not a physician, and that I should see a doctor regarding any changes I choose to make in my treatment plan. Carey Weiss will not be held liable for failure to diagnose or treat an illness, nor will she/he be liable for failure to prevent future illness. In addition, I have given Carey Weiss a complete and accurate account of any medical conditions that I may have and any medications that I am taking.
